Teacher Forms

Teachers please note that there are two tally forms, one for middle schools and another for high schools. Please use the tally form appropriate to your school level.

       ** Please note that ALL entry forms are to be sent to the following address. The deadline for the forms is February 15, 2013.**

• Tally form w/$10.00 per student. One check is preferred and made out to LRSF.
• Abstract form (see above) per student
  
• Rules Wizard print out AND ALL Intel ISEF forms required. These forms can be found at www.societyforscience.org. 
  
• ALL forms should be copied. Teachers to keep the original forms and LRSF receives copied forms. Please keep original forms until after our state competition. Students will need the original forms if attending ISEF in Pittsburgh. Additionally, if students decide on continuing their project next year, original forms are required.
  

NOTE: The MS Word Forms can be opened from Microsoft Word, filled in on the computer, saved and then printed. They can be modified later, if needed.

All forms filled in by hand or requiring a signature must be done in black ink and must be legible.
